COVID-19 Liability Waiver and Acknowledgement

Acknowledgement of Risk:
I acknowledge the highly contagious nature of the Coronavirus/COVID-19 and understand that the Centers for Disease Control and Prevention (CDC), along with other public health authorities, continue to recommend practicing social distancing to mitigate its spread.

Assumption of Risk:
I further acknowledge that KM Athletes cannot guarantee that I or my child will not be exposed to or infected by the Coronavirus/COVID-19 while participating in activities associated with KM Athletes Basketball Clinic. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may arise from the actions, omissions, or negligence of myself, my child, and others, including but not limited to KM Athletes staff, other participants, and their families.

Voluntary Participation:
By voluntarily choosing to attend or allow my child to attend services provided by KM Athletes, I acknowledge that I am increasing the risk of exposure to the Coronavirus/COVID-19. I agree to comply with all health and safety procedures implemented by KM Athletes to help reduce the spread of the virus while participating in the Basketball Clinic.

Health Attestation:
I hereby attest that:
– I, or my child, am not currently experiencing any symptoms of illness associated with COVID-19, such as cough, shortness of breath, difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
– I, or my child, have not traveled internationally within the last 14 days.
– I, or my child, have not traveled to a highly impacted area within the United States in the last 14 days.
– To the best of my knowledge, I, or my child, have not been exposed to anyone with a suspected or confirmed case of the Coronavirus/COVID-19.
– I, or my child, have not been diagnosed with Coronavirus/COVID-19 and have not been deemed contagious by state or local public health authorities.
– I, or my child, am following all CDC recommended guidelines to the best of my ability to limit my exposure to the Coronavirus/COVID-19.

Liability Release:
I hereby release, discharge, and hold harmless KM Athletes from any and all claims, demands, damages, costs, expenses, and causes of action that may arise from exposure to or infection by the Coronavirus/COVID-19. This release applies to myself, my child, my heirs, and any personal representatives. I understand that this release discharges KM Athletes from any liability or claim that I, my child, my heirs, or personal representatives may have against the Basketball Clinic in connection with any bodily injury, illness, death, medical treatment, or property damage that may result from participation in KM Athletes activities.

This liability waiver and release extends to all KM Athletes staff, coaches, volunteers, and affiliates involved in the Basketball Clinic.
