Informed Consent and Acknowledgement

I, the undersigned parent or legal guardian, hereby give my full and unconditional approval for my child’s participation in any and all activities organized by KM Athletes during the selected camp sessions. I acknowledge that my child’s participation in these activities is voluntary and that I have been fully informed of the nature, scope, and risks associated with the activities. I understand that these activities may include, but are not limited to, physical training, conditioning exercises, basketball drills, scrimmages, and other related activities.

In consideration of KM Athletes accepting my child’s participation in the camp, I hereby assume all risks and hazards incidental to the conduct of the activities, including but not limited to those arising from transportation to and from the camp, participation in the camp activities, and the use of equipment and facilities provided by KM Athletes. I understand that these risks may include, but are not limited to, injuries such as cuts, bruises, sprains, fractures, concussions, paralysis, or even death. I acknowledge that while KM Athletes and its staff will take reasonable precautions to ensure the safety of participants, they cannot eliminate all risks inherent in sports activities.

I agree to release, absolve, indemnify, and hold harmless KM Athletes, its officers, directors, employees, agents, volunteers, and representatives from any and all liability, claims, demands, or causes of action that may arise out of or in connection with my child’s participation in the camp. This release includes any claims arising from the negligence of KM Athletes or its staff, except to the extent that such claims are the result of gross negligence or willful misconduct.

In the event that my child is injured while participating in the camp, I authorize KM Athletes and its staff to seek medical treatment for my child as they deem necessary, including the transportation of my child to a medical facility for emergency care. I understand that I will be responsible for any medical expenses incurred as a result of such treatment, and I agree to provide proof of health insurance coverage for my child.

I further acknowledge and agree that my child’s participation in the camp is contingent upon their adherence to the rules and regulations established by KM Athletes. I understand that any failure by my child to comply with these rules may result in their dismissal from the camp without refund.

Additionally, I hereby grant KM Athletes the right and permission to use, copyright, publish, reproduce, and distribute my child’s name, voice, image, and likeness in video recordings, photographs, written materials, and audio-visual recordings made during the camp, for purposes including but not limited to promotional, advertising, and educational uses. I understand that these materials may be used in various forms of media, including but not limited to brochures, websites, social media, and other digital and print formats. I waive any right to inspect or approve the finished product or the use to which it may be applied.

I also acknowledge that KM Athletes reserves the right to make changes to the camp schedule, activities, and location, and I agree to accept such changes as necessary for the successful operation of the camp. I understand that in the event of inclement weather or other unforeseen circumstances, KM Athletes may cancel or reschedule camp sessions, and I agree that no refunds will be provided for such cancellations or rescheduling.

By signing this Informed Consent and Acknowledgement, I affirm that I have read, understood, and agree to the terms outlined above. I acknowledge that I am signing this document voluntarily and that I have the legal authority to do so on behalf of my child. I further agree that this document shall be binding upon me, my child, and our respective heirs, assigns, and legal representatives.
